I developed Dx64, a 64-bit windows application to debug PE64 formatted desktop applications on windows 8 or higher.
Dx64 implements a mode to show addresses relative to their module base.
Relative addresses and module numbers remain valid over different debug sessions.
You can find the documentation and the download link on my website .
As usual, You may have to deal with warnings about a suspicious download.
Move the unzipped Dx64.exe application to any location on your local computer.
You may make a shortcut on your desktop.
translators welcome.
I hope you enjoy my work.

The link redirects to https://sapplic.com/Dx64 manual (EN).html.

I couldn't find the source code mentioned or linked. Is it available?

No. Only a small part of Dx64 is written in fasm.
The major part is written in pseudo assembly which is only know by me.
The source must be compiled with my own private compiler program.
To give you an idea, I give you the source file (Dx64.rtf) here.
(zipped because this forum does not accept .rtf files).

Description: Example source of Dx64.
This is not fasm assembly code.

Download
