I found in TASM following mnemonics:

LEAVEW - 66 C9 / C9
LEAVED - C9 / 66 C9

ENTERW / ENTERD imm16,imm8 - 66 C8 .. / C8 ..

Tomasz,
maybe you add them to FASM?

There is no also FLDENVW/(D) instructions names

Yeah, it seems that here this analogy should also be followed. And with ENTERQ/LEAVEQ variants as well (though they would do nothing different from regular ENTER/LEAVE in the long mode, since 64-bit is default for stack operations and it's even not possible to encode 32-bit one - same situation as with PUSHQ/POPQ).
