Are there any custom controls written in FASM?

It would be especially helpful to view one written to be a dll and without the .if/.else stuff.

ASMEDIT by Tomasz on which fasmw is based on was coded purely in fasm, without .if stuff and with the great Tomasz's style of coding Smile. It is the best example I can think of

ASMEDIT was also released as DLL: http://board.flatassembler.net/topic.php?t=143
It's the old version there, though, I will put the new one there.
