Where are they defined? The AVX documentation I've been using only defines VPCLMULLQLQDQ, VPCLMULHQLQDQ, VPCLMULLQHDQ and VPCLMULHQHDQ.

I see, so it's an AMD variant of PCLMULHQHDQ and PCLMULHQHDQ. I wonder if they intentionally changed it (perhaps wanting it to be more consistent), or made a mistake when copying from Intel?

Since Intel was the one who designed AVX, I will for now stay with Intel's nomenclature - until we settle where this inconsistency comes from.
